Cycloserine is a broad-spectrum antibiotic originally isolated from Streptomyces orchidaceus and now produced synthetically. It is primarily used as a second-line antitubercular agent, especially for multidrug-resistant tuberculosis (MDR-TB) when first-line drugs (isoniazid, rifampicin) cannot be used due to resistance or intolerance.
Cycloserine works by inhibiting bacterial cell wall synthesis at two critical enzymatic steps: 1. Inhibition of Alanine Racemase — converts L-alanine to D-alanine (Cycloserine binds competitively and blocks this enzyme). 2. Inhibition of D-Alanine–D-Alanine Ligase (Ddl) — prevents formation of D-Ala–D-Ala dipeptide, required for cross-linking the peptidoglycan structure.
