Glimepiride is an oral antidiabetic medication belonging to the sulfonylurea class. It is primarily used to manage type 2 diabetes mellitus by stimulating insulin secretion from pancreatic beta cells. Chemically, it is a second-generation sulfonylurea with the molecular formula C₂₃H₃₂N₄O₅S. Glimepiride works by binding to the sulfonylurea receptor on pancreatic beta cells, leading to closure of ATP-sensitive potassium channels, cell depolarization, and subsequent insulin release.
Glimepiride works by stimulating the release of insulin from the pancreatic beta cells. It binds to sulfonylurea receptors on these cells, which leads to the closing of ATP-sensitive potassium channels. This closure causes the cell membrane to depolarize, triggering the opening of voltage-dependent calcium channels. The influx of calcium ions into the cells then promotes the release of insulin stored in secretory vesicles. By increasing insulin secretion, glimepiride helps lower blood glucose levels, making it effective for managing type 2 diabetes. It requires functioning pancreatic beta cells to work and is therefore not used to treat type 1 diabetes.
