Pyrazinamide is a key first-line antituberculosis medication used in combination therapy. It plays a vital role in shortening the duration of TB treatment by effectively targeting Mycobacterium tuberculosis residing in acidic environments such as macrophages.It is well recognized for its sterilizing activity during the intensive phase of TB treatment.
Pyrazinamide is converted into its active metabolite, pyrazinoic acid (POA), by the bacterial enzyme pyrazinamidase. POA accumulates in the bacterial cell, disrupting membrane transport and energy production, leading to bactericidal effects—especially in acidic conditions.
